Hope Community Services - Leonora

Keeping people in the community through continuity of care with general practice; People with multiple morbidities especially chronic co-occurring physical and mental health conditions; Services designed to meet the health of vulnerable and disadvantaged people including those of Aboriginal heritage; system navigation and integration to help people get the right services at the right time and in the right place; Capable workforce tailored to these priorities.  

Services provided

Drug and Alcohol Treatment; Mental Health service and support; Outreach to regional communities (including Laverton); Diversion through Leonora and Laverton court; Information and Education sessions (for the community and clients); Community prevention support and activities.

Target Groups

Adults and young people; Aboriginal people; People with co-occurring mental health, alcohol and/or drug concern; People from culturally and linguistically diverse population; People who identify themselves as Lesbian, Gay, Bisexual, Queer, Transgender, Intersex and Asexual (LGBQTIA)

Self referred; Service provider; GP and Hospital; Family and significant other; Court, Magistrate and Lawyers.
